CC Plates / A Arms - Install Question

Does anyone have pictures of the way the UPR CC Plates are suppose to install ? I think i have it correct, however, im a picture guy -

Also, When installing the a arm to the spindle, does the thick washer provided, go under the nut, and how tight does the castle nut have to be ?
