I've been looking through several different launchers and I don't see anything exactly like this but I'd guess it's using something like ADW, GO Launcher, Launcher Pro, or something like that. They all have a shit ton of customization to them.

It's pretty easy, just custom dock and icon images, hold down to change icon images in LP, etc.

They come out looking nothing like the screen shot.
