I can't recall who had the problem with their brass not holding primers. It was someone reloading .454 I believe. I actually ran into this recently and found a solution.

Both, really. Eventually the primers will literally fall out, but they start out as loose. Honestly, I had never run into this phenomenon because I rarely load a case more than twice just because I usually have a pretty high volume of cases. With .300blk I was getting sick of making cases so I reused the same set of 200 cases for a really hot load and it accelerated the phenomenon. What do you consider a loose primer pocket? Where the primer just falls out or the primer is physicaly loose? I had some that were super easy to press in, but they didn't fall out.
Anyways, to solve the problem you just have to bell the case head which forces material into the primer pocket mouth. Best way to do this is take a ball bearing (anywhere from 1/4" to 3/8") I find the larger ball bearings work. You need to make yourself a metal rod that will fit inside the case, so you can slip the mouth over the rod and down onto the head of the case, inside. Then with the primer pocket facing up while it's sitting on the rod you place the ball bearing on the pocket, and smash the mother loving hell out of it with a mallet (I'd recommend a sand filled one so the ball bearing doesn't go flying).
