I understand that you're a doctor, but I went to the doctors office a couple of times for some shit on my pinky toe and they kept saying they're "corns" and to just use the Dr. Scholl's stuff and it never worked. Finally I was sent to a specialist...this old dude that LOOKED crazy but when he first saw it and looked at the folder from my usual doc was like "Yep....they're warts." He prescribed me some cream, and after a couple of months of applying it every day they're gone and haven't been back in nearly 15 years. Planters wart. Treat it three times a day and get better ventilated shoes with better padding . Shoes designed to win a marathon are not shoes designed to train 25hrs a week . May I suggest you google "smart wool " socks . They fix all sorts of feet ailments , however they do not come with the blow job the price would suggest .

I am concerned, only because if it keeps getting more painful, it could affect my running. I need to get to a Dr. but it's tough when I'm always traveling.

It's neither concave, or convex. Just a hole, that feels hard a few mm's around the circumference. I think it could be a plantar wart, after doing some more reading.
